Appointments


To Make an Appointment

Therapy appointments are scheduled on a standing appointment basis so that you are assured of having appointments on a regular basis.  Frequency of sessions will be determined by Dr. Mattox and the patient/parents of the patient at the time of the first appointment.  When needed, frequency can be decreased or increased, pending availability in Dr. Mattox’s calendar.  There are a very limited number of after school appointments available for therapy.

For evaluations, the testing portion is scheduled during the day, typically on Tuesday mornings.  Other sessions associated with the evaluations are scheduled on Tuesdays, Wednesdays, and Thursdays.

New patients, PLEASE NOTE: New patient paperwork will be provided to you after scheduling, and it will need to be completed for each new patient prior to the first appointment.  It takes most people about 30-45 minutes to complete all of the forms.  You may bring the paperwork with you to the first appointment, or alternatively, you may mail the paperwork to the office well in advance of the appointment to ensure it arrives on time.  If you are unable to print the forms, please notify our office that you will arrive 45 minutes prior to your appointment and we will leave the forms in the waiting room for you to complete.

Previous patients: If you have been seen by Dr. Mattox in the past and are resuming services, she will let you know at the time of scheduling what paperwork, if any, is required.

Cancellation List for Testing/Evaluations: If you wish to be placed on a cancellation list, please complete your paperwork and send it in to the office (by mail, not fax).  Completed paperwork will be placed in order received, and those patients will be called in that order if an appointment becomes available.

Please bring the following to your first appointment:

  1. New patient paperwork (see Forms)
  2. Your insurance card, if you wish Dr. Mattox to file the charges to your out-of-network benefits
  3. A photograph of the patient – this will stay in the chart
